Questionnaire Objective
To identify:
i. Hazards in Manufacturing Environment
ii. Hazards mapping in terms of processes, machineries, and other parameters
iii. Drivers of Hazards
iv. Risk assessment activities in Manufacturing
v. Controlling Risks and its practices
vi. Current practices of HIRARC – Development and Implementation
vii. Issues and Challenges in conducting HIRARC
The input gathered from this will be the basis of developing a data analytic model to
produce a descriptive/analytic dashboard on HIRARC which will facilitate data-driven risk assessment,
making it more effective and accurate. SIRIM Berhad truly abide by the Personal Data Protection Act 2010
(Act 709) (“PDPA”) and shall ensure the Personal Data disclosed in relation to this questionnaire will be in
compliance with the PDPA.
Declaration of Data Privacy
The Personal Data you have entered will be collected, processed, and used by SIRIM Berhad for the purpose of a project between SIRIM Berhad and NIOSH. Insofar as is required for the project, the Personal Data may be transferred to NIOSH and the Personal Data may be analysed for reporting purposes (e.g. statistics).
By signing this Questionnaire, you hereby unconditionally agree that SIRIM Berhad and its related corporations, subsidiaries, associates, affiliates and joint development companies as well as their representatives (collectively, “SIRIM”) may collect, obtain, store and process your Personal Data that you have had provided in this Questionnaire for the purpose of the project.
You hereby unconditionally give your consent to SIRIM to:
a. Store and process your Personal Data;
b. Disclose your Personal Data to the relevant governmental authorities or third parties where required by law, regulatory body or for legal purposes.
For the avoidance of doubt, Personal Data includes all data defined in the PDPA and includes all data you had disclosed to SIRIM in this Questionnaire.
